GRADUATE STUDENT SUPPORT FORM

PLEASE DO NOT USE THIS FORM FOR SUMMER SEMESTER.  FACULTY HAVE BEEN EMAILED FOR SUPPORT INFORMATION.

 

GRADUATE STUDENTS

All ChBE graduate students must complete the online Graduate Student Support form each semester.  There are two main reasons for completion of this form:  1) to determine appropriate salary and project distribution, and 2) to assist the Academic Advisor in determining if a tuition waiver is appropriate.  Even ChBE students who are paid through a different department should complete this form.

FACULTY ADVISORS

After students complete the support form, the system will email the faculty advisor to review and approve the financial portion of the form.  Faculty members should enter specific project numbers and the percent distribution for a student’s salary.  Salary distribution must equal 100% in order to be accepted by the system.  If the project number is unknown, then the sponsor and project title should be provided in the comment section.

DEADLINES FOR SUPPORT FORM COMPLETION

Students and their faculty advisors must complete and approve the support forms by the following dates each semester.  These deadlines are driven by departmental and Institute deadlines such as financial aid in the case of fellowship tuition waiver requests.
Fall Term – July 15
Spring Term – November 30
Summer Term – March 31

The system will send out reminders to both the student and faculty advisor, once the student has submitted the form for each semester.  Reminders can only be stopped by completing the submittal and approval process.  Failure to complete the submittal and approval by the deadline may result in cessation of pay for the student.

NOTE:  if there are any changes to a student’s status between regular semester submittals, a revised form should be completed by the student and approved by the faculty advisor.  This off-cycle revision should be reported to the academic & finance office immediately upon completion of the form so that the data may be retrieved and processed.
